{"id":226647,"date":"2025-10-10T10:29:16","date_gmt":"2025-10-10T17:29:16","guid":{"rendered":"https:\/\/www.insidephilanthropy.com\/?p=226647"},"modified":"2025-10-16T11:07:05","modified_gmt":"2025-10-16T18:07:05","slug":"what-gates-is-backing-with-a-2-5-billion-womens-health-commitment","status":"publish","type":"post","link":"https:\/\/www.insidephilanthropy.com\/home\/what-gates-is-backing-with-a-2-5-billion-womens-health-commitment","title":{"rendered":"What Gates Is Backing with a $2.5 Billion Women\u2019s Health Commitment"},"content":{"rendered":"\n<div class=\"wp-block-genesis-blocks-gb-container gb-block-container\"><div class=\"gb-container-inside\"><div class=\"gb-container-content\">\n<p>About 830 women and adolescents die daily due to preventable pregnancy and childbirth complications, <a href=\"https:\/\/www.afro.who.int\/health-topics\/maternal-health\" target=\"_blank\" rel=\"noreferrer noopener\">according to the World Health Organization<\/a>. Ninety-nine percent of all maternal deaths occur in developing countries and over half occur in sub-Saharan Africa.<\/p>\n\n\n\n<p>To help upend those grim figures, in August, the <a href=\"https:\/\/www.insidephilanthropy.com\/find-a-grant\/grants-g\/gates-foundation\">Gates Foundation<\/a> called on its vast endowment to launch a $2.5 billion global commitment over five years aimed at expanding research that focuses on women\u2019s health and low-cost innovations to provide greater access to care. That\u2019s according to Dr. Ru-fong Joanne Cheng, the director of Women\u2019s Health Innovations at the Gates Foundation.\u00a0<\/p>\n\n\n\n<p>The investment, Gates\u2019 <a href=\"https:\/\/www.gatesfoundation.org\/ideas\/media-center\/press-releases\/2025\/08\/womens-health-funding-commitment\" target=\"_blank\" rel=\"noreferrer noopener\">largest<\/a> yet for women\u2019s health research and development, comes at a time when the United States has gone from being the top donor country in global maternal and child health, which the government has funded for more than a half-century, to negating vast swaths of such funding this year. The Trump administration\u2019s 2026 fiscal year budget request did not include any funding for bilateral maternal and child efforts, according to a<a href=\"https:\/\/www.kff.org\/global-health-policy\/the-trump-administrations-foreign-aid-review-status-of-u-s-global-maternal-and-child-health-efforts\/\" target=\"_blank\" rel=\"noreferrer noopener\"> report<\/a> by the Kaiser Family Foundation.&nbsp;<\/p>\n\n\n\n<p>In<a href=\"https:\/\/www.gatesfoundation.org\/ideas\/speeches\/2025\/06\/bill-african-union\" target=\"_blank\" rel=\"noreferrer noopener\"> a talk<\/a> Bill Gates gave in early June in Addis Ababa, Ethiopia, he alluded to the United States pulling the plug on its global giving. He also said that as he donates his entire fortune over the next 20 years, \u201cthe majority of that funding will be spent on helping you address challenges here in Africa.\u201d<\/p>\n\n\n\n<p>\u201cA lot of cuts are being made in foreign aid programs. Some of those cuts are being made so abruptly that there are complete interruptions in trials, or medicines are still sitting in warehouses and are not available,\u201d Gates said. \u201cAnd these cuts are something that I think are a huge mistake.\u201d<\/p>\n\n\n\n<p>The $2.5 billion global commitment is necessary because research into women\u2019s health is an area that has been largely ignored, said Dr. Anita Zaidi, president of the Gates Foundation\u2019s Gender Equality Division.<\/p>\n\n\n\n<p>\u201cThe male body was the default answer [in research],\u201d Zaidi said during a webinar. \u201c[Women\u2019s health has] been a blind spot in medical and bioscience research, with the result that only 1% of funding \u2014 that is, if we just leave out cancer \u2014 is directed to women\u2019s health R&amp;D for conditions that specifically affect women.\u201d<\/p>\n\n\n\n<p>In a recent<a href=\"https:\/\/www.newyorker.com\/news\/the-lede\/why-so-many-medicines-are-off-limits-during-pregnancy\" target=\"_blank\" rel=\"noreferrer noopener\">&nbsp;<em>New Yorker<\/em> article<\/a> on the dearth of research on pregnant women, Ruth Faden, the founder of the Johns Hopkins Berman Institute of Bioethics, said that one of the reasons that pregnant women haven\u2019t been included in medical research is the ongoing belief that they\u2019re \u201cfragile\u201d and need to be hidden away. \u201cWe need to protect women through research, not from research,\u201d she said.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What work Gates aims to prioritize with its new effort<\/h2>\n\n\n\n<p>The Gates Foundation&#8217;s commitment <a href=\"https:\/\/www.gatesfoundation.org\/ideas\/articles\/2-5-billion-womens-health-innovations\" target=\"_blank\" rel=\"noreferrer noopener\">will focus on<\/a> developing research and low-cost innovations around obstetric care and maternal immunization, contraception, sexually transmitted infections, maternal health and nutrition, and gynecologic and menstrual health for women in sub-Saharan Africa and South Asia, where women are the most burdened by these health issues, according to Zaidi.&nbsp;<\/p>\n\n\n\n<p>About half of all pregnancies around the world <a href=\"https:\/\/www.unfpa.org\/press\/nearly-half-all-pregnancies-are-unintended%25E2%2580%2594-global-crisis-says-new-unfpa-report\" target=\"_blank\" rel=\"noreferrer noopener\">are unplanned<\/a>. In response, the Gates Foundation will be supporting research into self-injectable patches that can provide six months of contraception, and a discreet birth control method.<\/p>\n\n\n\n<p>\u201cMany women in these communities want to have control over their reproductive health but do not use existing contraceptive methods because of male interference, concerns about side effects, or lack of access to options that meet their specific needs,\u201d wrote Cheng in a<a href=\"https:\/\/www.gatesfoundation.org\/ideas\/articles\/2-5-billion-womens-health-innovations\" target=\"_blank\" rel=\"noreferrer noopener\"> blog post<\/a>.<\/p>\n\n\n\n<p>The Gates Foundation is also focusing on STIs. Instead of having women wait for lab results, Gates will continue funding research into developing a low-cost, portable diagnostic tool the size of a soda can that women can use to learn within minutes whether they have a sexually transmitted infection. That technology, wrote Cheng, \u201ccould be a game-changer in low-resource settings, where access to STI testing is often limited or nonexistent.\u201d&nbsp;<\/p>\n\n\n\n<p>Similarly, the new funding will be expanding development around a low-cost, handheld, AI-powered ultrasound machine to let community health workers conduct ultrasounds and refer high-risk cases to specialists. Two-thirds of women who are expecting don\u2019t have access to ultrasound tests in their communities, according to a Gates Foundation<a href=\"https:\/\/www.gatesfoundation.org\/ideas\/science-innovation-technology\/future-womens-health-technology\/ai-ultrasounds\" target=\"_blank\" rel=\"noreferrer noopener\"> video<\/a>. \u201cBringing technology closer to women\u2019s homes is where we can make a difference,\u201d said Dr. Zahra Hoodbhoy, an assistant professor in the Department of Pediatrics and Child Health at A\u011fa Khan University in Karachi, Pakistan, in the video.<\/p>\n\n\n\n<p>Besides further developing and improving existing diagnostic tools, Gates aims to shore up efforts to study how unhealthy changes in the vaginal microbiome can provide clues to understanding pregnancy complications. It\u2019s supporting research like <a href=\"https:\/\/gcgh.grandchallenges.org\/grant\/evaluation-vaginal-microbiome-and-metabolomic-profiles-adverse-pregnancy-outcomes-kenyan\" target=\"_blank\" rel=\"noreferrer noopener\">a study<\/a> led by Dr. Moses Obimbo Madadi,<strong> <\/strong>a clinician-scientist at the University of Nairobi in Kenya.<\/p>\n\n\n\n<p>The study, which began in 2021, involves collecting clinical data and vaginal samples from more than 1,200 pregnant women in Kenya and analyzing microbial content to get a complete profile of the vaginal microbiome. The data, in turn, will be used to develop AI-assisted prediction models that could help provide early screening tools to identify at-risk pregnancies.<\/p>\n\n\n\n<p>Madadi\u2019s idea for the research stemmed from a patient who was plagued with recurring vaginal infections that caused her such physical and emotional discomfort that she avoided going out much in public. While longer-term treatment was available, for lower-income patients like her, who are seen at a public hospital, the cost was out of reach, Madadi said.<\/p>\n\n\n\n<p>\u201cSo when I saw this and how she was affected physically and psychologically, I thought, supposing she gets pregnant, how would recurring infections affect her pregnancy?\u201d Madadi said. \u201cIt became very personal for me.\u201d<\/p>\n\n\n\n<p>As part of Gates&#8217; new commitment, it is also providing funding to the Massachusetts-based Comanche Biopharm, which is working on a medication to mute a key protein that triggers preeclampsia.<\/p>\n\n\n\n<p>Preeclampsia and eclampsia, a blood pressure disorder that strikes women during pregnancy or after they give birth, is a leading cause of maternal and infant mortality worldwide, according to the American College of Obstetrics and Gynecologists. Its billions will be even more vital in an era of diminishing public sector support for global health, and while Bill Gates\u2019 own criticisms of Trump administration policies <a href=\"https:\/\/www.insidephilanthropy.com\/home\/as-trump-guts-usaid-is-it-time-for-bill-gates-to-get-angry\">have often been muted at best<\/a>, he has registered his displeasure at, for instance, the evisceration of USAID. He has also clapped back at the administration\u2019s dangerous antivaccine stances, <a href=\"https:\/\/www.insidephilanthropy.com\/home\/bill-gates-defends-his-legacy-on-vaccines-and-highlights-the-threat-to-global-health\">recommitting to funding Gavi, the Vaccine Alliance<\/a> with $1.5 billion over another five years.<\/p>\n\n\n\n<p>There are some hopeful signs the Gates Foundation will continue scaling up efforts the federal government has abandoned \u2014 ideally alongside other funders. This latest women\u2019s health commitment follows another big move: Alongside other global health grantmakers, Gates is also backing the Beginnings Fund, a \u201cnearly $600 million\u201d commitment for maternal and infant health <a href=\"https:\/\/www.insidephilanthropy.com\/home\/the-beginnings-fund-good-news-for-mothers-and-children-as-aid-funding-collapses\">IP wrote about in June<\/a>.&nbsp;<\/p>\n\n\n\n<p>Separately from the Gates Foundation commitment, three other members of the Gates clan \u2014 Melinda French Gates and daughters Jennifer and Phoebe \u2014 recently joined ICONIQ Impact and a range of other donors <a href=\"https:\/\/www.iconiqcapital.com\/impact\/insight\/whc-co-lab-launch\" target=\"_blank\" rel=\"noreferrer noopener\">to launch the Women&#8217;s Health Co-Lab<\/a>, a collaborative fund focused on global health inequity for women and girls.<\/p>\n\n\n\n<p>\u201cI think one thing that\u2019s different about the $2.5 billion is that we\u2019re approaching it as a moment to catalyze the entire community, the global ecosystem around women\u2019s health research and development,\u201d Cheng said. \u201cWe\u2019re saying, we think women\u2019s health is worth investing in, and we want you all to come in, too, and it\u2019s really a call to action, to highlight the urgency or the need. It\u2019s not just that we\u2019re investing in it and that\u2019s it. It is truly a collective response that\u2019s going to be required to move the needle here.\u201d<\/p>\n<\/div><\/div><\/div>\n\n\n\n<div class=\"wp-block-genesis-blocks-gb-container
